TOURNAMENT WAIVER: I, the team representative (coach or team manager) of the registrant (team), a minor, or adult registrant of legal age, agree that I and the registrant will abide by the rules of the East Penn Soccer Club, and its affiliated organizations and sponsors. Recognizing the possibility of physical injury and/or death associated with soccer and its related activities and in consideration for accepting the registrant for its soccer programs and activities (the Programs), I hereby release discharge and/or otherwise indemnify the East Penn Soccer Club, their Board of Directors and staff, its affiliated organizations and sponsors, their employees and associated personnel, including the owners of the fields and facilities utilized for the programs, against any claim by or on behalf of the registrant as a result of the registrant’s participation in the programs, and/or being transported to or from the same, which transportation I hereby authorize. I further grant the East Penn Soccer Club Parties the right to use the player’s name, pictures and /or likeness in printed, broadcast, and other material concerning the Programs provided such use is related to the player’s status as a participant in the Programs.
